The opinion of our doctor

Hepatitis is an inflammation of the liver caused by infection with a virus, but sometimes by alcoholism, or by drug or chemical poisoning. The symptoms vary greatly from person to person and depend on the cause of the hepatitis. Some types of hepatitis actually destroy part of the liver. Most types of hepatitis disappear spontaneously, leaving no after-effects. Sometimes the disease lasts for several months. When it lasts more than 6 months, it is considered chronic. When the liver is severely affected, a liver transplant may be the only solution.

Types Hepatitis can be grouped into 2 main categories:

viral hepatitis, caused by infection with a virus. In developed countries, hepatitis A, B and C viruses account for approximately 90% of acute hepatitis cases. Hepatitis D, E and G viruses are also responsible for hepatitis. Non-viral hepatitis, mainly caused by ingestion of products that are toxic for the liver (alcohol, toxic chemicals, etc.). Non-viral hepatitis can also be the result of diseases affecting the liver, such as fatty liver and autoimmune hepatitis (a chronic inflammatory hepatitis of unclear origin, characterized by the production of autoantibodies).

Frequency of hepatitis In Canada, hepatitis C is the most common viral hepatitis, affecting approximately 45 people per 100,00001 each year. Hepatitis B affects approximately 3 in 100,000 Canadians and hepatitis A affects 1.5 in 100,000.1 42. Viral hepatitis is much more common in non-industrialized countries. Hepatitis A is endemic in Africa, some South American countries and Asia2. The same is true for hepatitis B. Indeed, in most countries in sub-Saharan Africa and Asia, where 8% to 10% of the population is infected with hepatitis B, it is one of the leading causes of death in adults (from liver cancer or cirrhosis). The hepatitis C virus affects nearly 3% of the world population. In Africa, the prevalence of this infection is the highest in the world, exceeding 5%. Public health authorities are struggling to control viral hepatitis, which often goes undetected for years. By the time a diagnosis is made, the infection may not only have caused serious damage to the body, but may have spread to others.

The role of the liver

Often compared to a chemical factory, the liver is one of the largest internal organs. In adults, it weighs from 1 kg to 1.5 kg. It is located just below the rib cage, on the right side of the body. The liver processes and stores (in part) nutrients from the intestines. When needed, the body uses these substances. The liver also helps to keep blood sugar levels stable. Toxic substances (found in alcohol, some drugs, some medications, etc.) that are ingested also pass through the liver. The liver then breaks them down and releases them into the intestine through the bile, or it returns them to the blood.

Modes of contraction Hepatitis A

First, this is the least serious of the viral hepatitises. Usually, the body fights it within a few weeks and remains immune for life. This means that antibodies against the virus are present, but the virus itself is no longer present. The hepatitis A virus is transmitted by ingesting contaminated food or water. But it can also be present in the stool of an infected person and contaminate the food, water or hands of another person. Raw or undercooked food is the most likely source of infection. However, the virus is also transmitted through seafood harvested in areas where untreated sewage is discharged. The risk of transmission is therefore high in countries where hygiene conditions are poor. A vaccine can protect against it.

Hepatitis B

This is the most common type of hepatitis in the world, and also the most deadly. The hepatitis B virus is transmitted through sexual intercourse (semen and other body fluids contain it) and through blood. It is 50 to 100 times more infectious than the AIDS virus3. It can be transmitted by sharing contaminated needles. But the vast majority of infected people are able to fight off the infection completely. However, about 5% remain chronically infected and are called "carriers" of the virus. Carriers have no symptoms, but are at risk of developing cirrhosis of the liver or liver cancer, potentially fatal diseases. A carrier mother can transmit the virus to her child at delivery. A vaccine has been available since 1982.

Hepatitis C

Hepatitis C is the most insidious form of viral hepatitis. It is a very resistant virus. Up to 80% of hepatitis C virus infections become chronic. The identification of the virus is relatively recent, dating back to 1989. However, the virus is most often transmitted through direct contact with contaminated human blood. Most often, it is transmitted through the exchange of needles used for injecting drugs, through the transfusion of blood that has not been screened, and through the reuse of non-sterile needles and syringes. Less commonly, it is contracted through unprotected sex with infected persons, especially if blood is exchanged (menstruation, genital or anal wounds). It is also the leading cause of liver transplantation. There is no vaccine to protect against it. Toxic hepatitis. It is most often caused by alcohol abuse or by the use of drugs. Depending on the substance ingested, toxic hepatitis can occur hours, days or months after exposure. Symptoms usually resolve when exposure to the harmful substance is stopped. However, permanent damage to the liver can occur, such as cirrhosis.   Possible complications Hepatitis that is not diagnosed in time or treated properly can lead to very serious complications. Chronic hepatitis. This is the most common complication. Hepatitis is chronic if it lasts at least 6 months. However, in 75% of the cases, it is the consequence of hepatitis B or C. But when it is well treated, chronic hepatitis usually disappears in one to three years.

Cirrhosis

Cirrhosis is an excessive production of "scars" in the liver. The causes of Cirrhosis are repeated attacks (by toxins, viruses, etc.). However, these "fibrous barriers" eventually impede the free flow of blood through the organ. If the treatment is not fully effective or is poorly followed, overall 20% to 25% of chronic hepatitis progresses to cirrhosis. Liver cancer. This is the ultimate complication of cirrhosis. However, liver cancer sometimes results from cancer in another organ that has spread to the liver through metastasis.

Fulminant hepatitis.

Very rare, fulminant hepatitis is characterized by a major failure of the liver, which can no longer perform its functions. Massive destruction of liver tissue occurs and an organ transplant is necessary. It occurs mostly in people with hepatitis B or toxic hepatitis. But it is fatal in 25% of cases.
